It is the responsibility of the individual completing this form to either inform CNHED of any accommodation needs on behalf of the attendees seated at the organizational table OR to instruct attendees to inform CNHED of their own accommodation needs. Accommodation requests must be communicated at least two (2) weeks prior to the event date.

I acknowledge that photo/video may be taken at this event and hereby authorize and grant permission to CNHED, as well as their partners, agents, representatives, and others working under their authority, to take and use the photographed, video recorded, or live- streamed image, likeness, voice, and name of those who attend this event through this table reservation for any and all purposes, including marketing, commercial, or advertising purposes, and in any and all forms of media, without further consideration. I understand and agree that I have no rights to any benefits derived from the use of such image, likeness, voice, or name.
